Billing Policy

Introduction

This Billing Policy is incorporated into and subject to the FormVeritas Terms of Service. It governs purchases, the White Label Reports subscription, payments, refunds, chargebacks, and billing records when you buy access to the FormVeritas software and services from One Big Media Company LLC.

What We Sell

FormVeritas currently offers the following. Prices are in U.S. dollars and do not include any applicable taxes.

• Free: $0. Includes 1 audit. No payment method is required. We may require you to confirm your email address before you can use the free audit.
• USCIS Form Review: $197, charged once. Includes 40 audits and team seats, so you can invite team members to a shared account.
• Audit Packs: one-time top-ups added to your current billing account: 20 audits for $79, 50 audits for $149, 100 audits for $249, or 250 audits for $499.
• White Label Reports: $99 per month, billed as an automatically renewing monthly subscription. It removes "Powered by FormVeritas" branding and applies your agency's name and logo to exported reports. It is available to eligible paid shared accounts and can be bought and cancelled only by the account owner.

The prices shown in the app or at checkout when you buy are the prices that apply to that purchase.

One-Time Purchases and Audit Packs

USCIS Form Review and Audit Packs are one-time purchases. Each is charged once when you check out and does not renew. Audits are added to the billing account you are using when you buy, and they belong to that account.

Audits may be base audits from a plan or pack, or bonus audits that we grant, such as the free audit. Purchased audits do not expire while the account remains open. Audits have no cash value, cannot be transferred to another account, and are forfeited if the account is deleted.

White Label Reports Subscription

Your subscription starts when your first payment succeeds. Receipts and invoices are provided electronically.

Failed payments: If a renewal payment fails, Stripe may retry the charge. If the payment cannot be collected and the subscription ends, White Label branding is turned off for the account.

Cancelling White Label Reports

You can cancel White Label Reports at any time. Only the account owner can cancel it.

• From the Billing tab: in the FormVeritas app, open the Billing tab and choose to cancel White Label. Cancellation takes effect immediately. White Label branding is turned off right away, and you will not be charged again.
• From the billing portal: if you cancel through the Stripe billing portal and the portal offers cancellation at the end of the current billing period, White Label stays on until that period ends, and then turns off. You will not be charged again.
• By email: you can also ask us to cancel by emailing [email protected] from the account owner's email address. We will cancel it as soon as we have confirmed the request.
• Account deletion: deleting the account immediately cancels any active White Label Reports subscription.

Cancelling does not refund the current month, and we do not give partial-month refunds, except where a refund is required by law. Reports exported after White Label ends use standard FormVeritas branding. Cancelling White Label Reports does not affect your plan or your audits.

Payment, Taxes, and Chargebacks

Payments are processed by Stripe. FormVeritas does not receive or store your full card number. We will charge your chosen payment method for any paid transactions, including applicable taxes according to the tax jurisdiction in which the transaction takes place. You agree to receive all invoices and receipts in an electronic format.

We understand there may be occasions where you do not recognize a charge. You agree to contact us at [email protected] before initiating a chargeback or dispute with your bank or card issuer.

Refunds and disputes: If a payment is refunded or reversed through a dispute, we may remove the unused audits bought with that payment, and we may flag or restrict the account while we review it.

Collections and Account Suspension: If you fail to pay Service Fees on time, or if a chargeback is initiated, we reserve the right to immediately suspend your account. If you make a card payment and later dispute a legitimate charge without merit, we reserve the right to permanently prohibit you from using our Services and submit compelling evidence to refute the invalid chargeback request.

Final Sale Policy

Price Changes

We may change our prices. A price change does not affect purchases you have already completed. For the White Label Reports subscription, we will notify you by email before a new price applies to your next renewal, and you can cancel before it applies.

Termination and Suspension

If you fail, or we suspect that you have failed, to comply with any of the provisions of our Terms of Service (including our Acceptable Use Policy), we may, with or without notice to you, except where notice is required by applicable law: (i) terminate your account; (ii) terminate your license to the Product; and/or (iii) preclude your access to the Services. If we terminate an account, we will also cancel any active White Label Reports subscription so that it is not charged again. No refunds will be provided for accounts terminated due to Terms of Service violations, except where a refund is required by law.

Dispute Resolution and Governing Law

As detailed in our Terms of Service, any dispute, claim, or controversy arising out of or relating to financial transactions, billing records, chargebacks, or this Agreement shall be determined by binding individual arbitration in the State of Delaware, subject to a Class Action Waiver. This Agreement shall be governed by and construed in accordance with the laws of the State of Delaware, United States, without regard to its conflict of law provisions.
